PARENTAL PERMISSION:In registering my child, I understand that soccer is a contact sport and that injuries can and do occur. I hereby attest that I am this child's legal guardian and I have the authority to grant this permission for my child to play soccer. I further affirm that my child is physically able to play sports, that permission is given to administer emergency care, that I hold completely harmless all coaches, referees, volunteers, administrators and helpers of this program and that I hereby accept all financial responsibility for any medical care needed.

This spring, four-year-olds through eighth graders will practice and play on Mondays
and Fridays. That will allow us all to support our high school Tigers and Lady Tigers,
who are playing mostly on Tuesdays and Thursdays.
